The Medicine Center Patient Care Program is our process designed to streamline your pharmacy experience into one single visit and to make you feel entirely cared for.

  • We make the calls to your prescriber to make sure you always have refills on your medications and assist to resolve any insurance issues in advance of you needing your prescriptions.

  • You will receive a phone call (or text) each month from a pharmacy team member to review your medications, so that we only fill the prescriptions you need.

Medicine Center Patient Care Program focuses on you, the patient, by making you feel entirely cared for. One of the best ways that our pharmacists can help support you and your health is to make sure you have the medication you need when you need it. In the case of diabetes, hypertension, and high cholesterol, the studies have shown that patients who take their medications as prescribed achieve the intended effect of the medication and reduce the risk for increased medical care and hospitalizations. While most patients believe they are taking their medications as prescribed, national data reveals that 1 in 5 patients are not taking their medications as prescribed, which as a result, reduces the effect of the medication, increases the need for medical care, and increases the risk for hospitalization.

How Does Our Model Make a Difference?

The Medicine Center Patient Care Program helps to improve adherence in multiple ways:

  1. It reduces "gaps in therapy" by prompting your pharmacy team to review your prescriptions in advance to identify the need for refills from the doctor, resolve insurance issues, and order in your medication supply.

  2. It prompts a monthly communication between you and the pharmacy team to review your medication to identify changes and discuss your medications. This call helps us to identify when your medications have been stopped by your doctor, when you have stopped medications on your own, or when your directions have changed.

  3. It will eliminate out of stock situations and multiple trips to the pharmacy each month.

  4. We can arrange your pickup or home delivery date in advance to avoid running out of your medications.

  5. In our opinion, this helps to provide the HIGHEST LEVEL of PATIENT CARE in ensuring our patients are not only getting their medications, but also get education on medication therapy, and have the opportunity to discuss any concerns with their pharmacist.
